If you need copies of your records for yourself, or sent to other healthcare providers, we'd be happy to help. The department is open for non emergent release of information requests Monday through Friday 8:00 a.m. to 4:30 p.m. For those of you with Advance Directives concerning end of life issues, we welcome any advance directive information that you may have prepared.
